Target Audience: This program is designed for health care professionals: MDs, NPs, PAs, nurses providing services for children and adolescents in NYS School Based Health Centers. This conference will address issues that affect the patient, provider, school staff and SBHC staff.

Instructions for completing post-test and evaluation
 – Participants must register for and participate in the enduring activity (watch the webinar button) before taking the post test.
– Upon completion of the activity, participants have opportunity to take a test to receive CME credit (take test now button).
– In order to obtain credit participants have to complete course evaluation and post test.
– Pass rate of 60% is required to obtain CME credit. Participants are free to retake the test multiple times to pass.
– A course evaluation form can be obtained at http://www.upstate.edu/cme/. Click on “Forms and Resources”, click on “Evaluation”, download the form and send to cme@upstate.edu as an attachment with the course name HPV SBHC GUIDE in the subject line to receive credit.
